This screen is the first stage in initiating Performance Review for an individual employee.

Field Information
Performance Document*
This is the Performance Form to be submitted based on the following criteria.
Period Start and End Date*
The dates selected in the Start and End Date fields are used as the review period which is displayed on the selected review form.
Example
Employees are reviewed for the Period between May 2011 to April 2012.
ID Number*
This is the single employee number that will receive the Performance Document. Initially, the screen caters for 5 individual ID Numbers. If more are needed, use the More Rows icon.
Once the fields have been filled out, you may click Run Process to initiate the review.
